Affiliation Forms
All affiliates must be registered with Hockey Alberta on a HCR Roster and must be added as an affiliate prior to them playing in the CAHL. Use this link to submit those affiliates for review
Affiliation Approval form for all Players from outside of the CAHL
The CAHL requires notification of use of all affiliate players prior to them playing in a game. This is to allow website changes to allow the scoring of the game properly on the websites and for monitoring purposes. Please complete the following on line form to provide notification of the use of the player(s) each time you use them. You will receive a copy of your submission, this is your confirmation the notification is completed.
